ConnextCare Parish is a patient-centered healthcare practice providing Oswego and Northern Onondaga County residents with a variety of comprehensive healthcare and related services. The center is operated by ConnextCare, a private, federally funded non-profit organization governed by a volunteer Board of Directors.

Our Parish office has an intended closure on May 30, 2025. Below are Frequently Asked Questions regarding this transition.
Q: Why did the Parish location close?
A: The closing of the Parish site is part of a broader initiative to enhance the delivery of its services. By streamlining our resources through the consolidation of the Parish site into our larger network, we are better positioned to offer increased access to care – such as expanded acute care services with walk-in hours supported by a team of providers.
Q: Where is the closest ConnextCare location?
A: Three of ConnextCare’s other locations (Central Square, Mexico and Pulaski) are less than 12 miles from the Parish practice.
Q: Will I be able to see the same provider?
A: Providers and staff members associated with the Parish location will be offered the opportunity to transition to another health center within the ConnextCare system, therefore you can choose to follow your provider to the new location.
Q: Do I have to call and change my appointment scheduled after May 30th?
A: If you already have an appointment scheduled in the Parish office after May 30th, ConnextCare staff will contact you to assist with rescheduling this to an alternate location. You do not need to call our office and request to reschedule.
Q: Will I still have access to my medical records?
A: Your electronic health record will remain securely stored within the ConnextCare database and will be available to you and your provider, when requested.
Q: Will my insurance be accepted?
A: ConnextCare accepts the same insurance across all of its locations.
