ConnextCare Central Square is a patient-centered healthcare practice providing Oswego and Northern Onondaga County residents with a variety of comprehensive healthcare and related services. The center is operated by ConnextCare, a private, federally funded non-profit organization governed by a volunteer Board of Directors.

ConnextCare Central Square is located at 3045 East Ave, Suite G400, in Central Square, New York. When arriving, please take the driveway on the right hand side of the parking lot to the backside of the building. You will enter through the lower level and follow signs for the ConnextCare suite.
